Personalized Heart Health Assessment 
 
Prairie Heart Institute’s Personalized Heart Health Assessment is an individualized report about your cardiac health that gives you the tools you need to take better care of yourself. The Assessment is a physical exam and complete laboratory analysis that provides detailed information about the heart and possible indicators of cardiac disease.

Participants receive a 45-minute health assessment conducted by a Prairie Heart Institute cardiac nurse practitioner. Assessments include: body mass index , fasting glucose, blood pressure and fasting total lipid panel. A physical exam measures the function of the heart, lungs, carotids (arteries in the neck that carry blood to and from the brain) and the extremities.

After the results are reviewed by the nurse practitioner, recommendations are given to the participant and communicated to the individual’s primary care physician. If the physical exam shows signs of heart disease that need to be addressed immediately, a referral is made to a Prairie Heart Institute cardiologist — and appointments are guaranteed within one week.

For participants who do not require immediate medical attention, but do need resources to improve cardiovascular health, a comprehensive plan is created. Medical management of lipids, blood pressure or glucose levels is available at Prairie Heart and the Center for Diabetes and Metabolic Health.

Individuals over the age of 14 who want to learn more about their heart and how they can take better care of themselves should consider taking the Assessment.

Fee ranges from $50 - $125, depending on whether labs need to be drawn, and is billable to insurance. If further services are required, additional costs will vary. Financial assistance may be available.
