Acupuncture is used to treat energy imbalances that often result in pain and illness. During treatment, our licensed acupuncturist inserts fine, flexible, disposable needles at specific energy pathways that impact muscles, nerves and organs. This helps restore balance and enhances health and wellbeing.

If acupuncture is a covered service under your insurance plan the Center for Living
at St. John’s Hospital will bill your insurance company for you.
It is your responsibility to call your benefits department to confirm the service
is covered. It is also important you let your insurance company know the
provider of these services is St. John’s Hospital.
Once you know that your insurance reimburses for the service, we will need the following:
- an order from your physician with an ICD diagnosis code(s)
- you must pre-register as an outpatient
Please call us at 544-LIVE (5483) to schedule your appointment and/or for help in
completing the registration process.
Click on each image to download the two intake forms needed for
the initial acupuncture appointment.
Do not MAIL forms, BRING them to the first visit.
